Every portfolio and initiative has side panels on its Overview tab. It keeps the context around the work in one place: what could go wrong (Strategic Risks on a portfolio, Risks on an initiative), what was decided and why (Decisions), and the conversation and documents around it (Comments, Files and History)
Log a risk
Open the portfolio or initiative and find Strategic Risks (portfolio) or Risks (initiative) in the right-hand panel;
Select + Add. The Log a risk panel opens;
Fill in the fields and select Log risk.
Field | What to enter |
Title | Required. A short name for the risk. |
Description | What the risk is and what would trigger it. |
Severity | A slider from 1 to 5, set to 3 by default. The risk tile shows it as a label, for example Medium for 3. |
Probability | A slider from 0% to 100%, set to 50% by default. |
Impact areas | Select any that apply: Cost, Schedule, Scope, Quality, Regulatory, Reputational. |
Mitigation plan | Your plan to reduce or remove the risk. |
Status | Open (default), Mitigating, Closed or Accepted. |
Owner user ID | Optional. The ID of the person responsible. |
Due date | When the risk should be resolved. |
Risk statuses
Status | Use it when |
Open | The risk is identified and not yet being worked. |
Mitigating | Someone is actively working the mitigation plan. |
Closed | The risk has passed or been removed. |
Accepted | You've decided to live with the risk rather than mitigate it further. |
Review and update risks
The risks tile lists open risks with their severity and status labels.
Select Show all to open the full list;
Use the actions menu (⋮) on a risk to Edit or Delete it. Edit opens the same fields;
Select Save changes when you're done.
Record a decision
The decision log is a permanent record of the material choices made on a portfolio or initiative: what was decided, why, and what else was considered. It is append-only, so once a decision is saved its title, rationale, counterfactual and alternatives can't be changed. To override a past decision, record a new one.
Find Decisions in the right-hand panel and select + Add. The Record decision panel opens.
Fill in the fields and select Record decision.
Field | What to enter |
Title | Required. For example, "Approve initiative I-42". |
Rationale | Required. Why this was the right call. |
Counterfactual | Optional. What was projected if you hadn't made this call. |
Alternatives considered | Select Add alternative for each option you weighed, with a Title and Why rejected. |
Revertible | On by default. Switch it off to lock the decision permanently (uncheck to lock this decision permanently). |
The Decisions tile shows recent decisions, each with who captured it and when. Select Show all to open the Decision log. It shows each decision with its rationale and alternatives, lets you filter by Team under Filters, and has its own + Record decision button.
Existing Decisions
Open a decision to edit it. Only two things can change:
Evidence refs (JSON): a free-form JSON object that links the source records behind the decision, such as Slack messages or Jira tickets.
Reverted: the switch shows Active while the decision stands. Switch it on (Reverted (set on save)) and select Save changes to mark the decision as reverted. The tile and the Decision log then show Reverted.
When to log a decision
Record a decision whenever:
funding is approved, denied or reallocated
scope or priority changes
a risk is formally accepted rather than mitigated
capacity is rebalanced between initiatives
a portfolio or initiative is put on hold or cancelled
goal alignment changes

Select the Comments tab at the bottom of the right-hand panel:
Type in the comment box (Add a comment… use @ to mention). Use @ to mention a teammate.
Select the send icon.
Comments support basic formatting (bold, italic, underline, lists and links). Select Reply under a comment to answer in its thread.
Activity history
The History tab lists changes to the portfolio or initiative automatically, newest first, with who made each change and when. It includes risks and decisions being added or updated, for example, an added risk or an updated risk.
Files
Select the Files tab to attach documents such as business cases, steering-committee decks or vendor quotes.
Drag a file onto the drop area, or select the area to choose a file. Files can be up to 50 MB: PDF, images, Office documents, MP4 or ZIP.
